As a covered entity, as defined under the Health Insurance Portability and Accountability Act (HIPAA) of 1996, as amended, Kennedy Health System ("Kennedy" or also referred to in this Notice as "we", "us" or "our"), is required by law to maintain the privacy of your protected health information. This Notice of Privacy Practices (this "Notice") sets forth our obligations and your rights regarding the use and disclosure of your protected health information. Protected health information is individually identifiable health information that Kennedy or its business associates maintain or transmit in any form or medium, including verbal conversations and written or electronic information.
Personal information sent to Kennedy Health System via e-mail or via form submission on our web site will be used to respond to your message or request only. We will not share your information with other organizations without your written permission. The Kennedy Health System's Web site has links to other health information sources. If you choose to visit a link to another site via our site, you are subject to the privacy policies of that site.
Kennedy Health Care Foundation Donation Refund Policy
For donations made using credit cards, if the donor has made an error in making a donation or has a change in mind about their contribution, we will honor their request for a full refund if made within 15 days of their donation. To request a refund, the donor must contact the gift processing department.
